Who May Qualify
You may qualify if you have Medi-Cal through Health Plan of San Joaquin (HPSJ) or Anthem Medi-Cal, and you:
No Out-of-Pocket Cost
• Are experiencing homelessness, at risk of losing housing, or fleeing domestic violence
• Often use the emergency room or hospital, or live with serious mental illness or substance use challenges
Enhanced Care Management is a Medi-Cal benefit. Eligible members pay nothing.
• Recently left jail or prison, or are pregnant or recently had a baby
• Are at risk of needing nursing home care, or are a child in CCS (California Children's Services) or child welfare
Each family member is checked separately. We serve San Joaquin, Stanislaus, and Sacramento counties. This program is for ongoing support over several months, not one-time needs like getting glasses or signing up for CalFresh. It is not an emergency service. You may not qualify if you already get Enhanced Care Management from another provider, your main insurance is not Medi-Cal, or you are in a Medicare D-SNP plan or certain programs such as PACE, MSSP, or the Assisted Living Waiver.

Will I keep my current doctors?
What if I already receive other assistance?
Yes. Screening does not change your Medi-Cal coverage or your doctors.
Many programs, like CalFresh and IHSS, can continue. Some programs, such as PACE, MSSP, or certain waivers, may mean you don't qualify. The screening checks this.
Tulong Healthy Communities Corporation (THCC) is a nonprofit community partner. We help Medi-Cal members find out if they may qualify for Enhanced Care Management (ECM) and Community Supports, and we connect them with a Medi-Cal care management partner that provides these services. ECM is available at no cost to eligible Medi-Cal managed care plan members. THCC does not provide medical care, and this program does not replace emergency care. If you are having a medical emergency, call 911 right away. For a mental health crisis, call or text 988.
